Please PRINT the form attached with this announcement, COMPLETE, and INCLUDED the supporting documents as proof of the portfolio needed for the assessment. The forms and documents must be collected in the D-3 FFUP Study Program Room on the 1st floor to Mrs. Tera Soilina.

The forms and supporting documents must be collected no later than Friday, August 9, 2019 at 16.00 WIB.

The forms in question are as follows:

• Past Learning Recognition Application Form (RPL) (Appendix 2, Sections 1, 2, and 3).
• Past Learning Recognition Self-Assessment Form (RPL) (Appendix 3).
• RPL Program Student Personal Data Form.
