Announcement to Prospective Students of the Pharmacist Professional Education Study Program, Faculty of Pharmacy, Pancasila University, Based on the results of the selection tests conducted on Tuesday and Wednesday, July 14–15, 2025, the results of the Pharmacist Professional Education Study Program Selection Test Participants are as follows:

Students who have been accepted are requested to immediately re-register and complete the specified administrative requirements from Wednesday, July 24, 2025, to Tuesday, August 5, 2025, at 2:00 p.m. If you do not submit and upload the files by the specified time, you will be considered to have withdrawn.

Re-registration Documents for Pharmacists

1. Fill out the registration form and fill out the statement letter for the Pharmacist Professional Study Program (can be downloaded via the link http://s.id/FORMDAFTARULANGAPT )
2. Certified copy of Bachelor’s degree certificate: 1 copy
3. Certified copy of Bachelor’s degree transcript: 1 copy
4. Two 4×6 black and white photos
5. Two 3×4 color photos with red background
6. Certified copy of birth certificate: 1 copy
7. Certified copy of family card: 1 copy
8. Health certificate from a doctor.
9. Certificate of color blindness, speech impairment, and hearing impairment (original) from a doctor and drug-free certificate from the competent authority.
10. Photocopy of the accreditation certificate of the faculty/undergraduate program from the original Pharmacy College.
11. Proof of acceptance

The documents were submitted to the Academic Department of the Faculty of Pharmacy, Pancasila University, and uploaded to https://s.id/DAFTARULANGAPT76 no later than Tuesday, August 5, 2025, at 2:00 p.m. WIB.

*Please name the document to be uploaded as NAME_3 LAST NUMBER OF REGISTRATION NUMBER_FILE TYPE. Example: FULAN_249_DIPLOMA.

Orientation for new pharmacy students on Friday, August 15, 2025, at 9:00 a.m.
Classes will begin on Monday, August 18, 2025.
